To determine if symptoms displayed during or after a transfusion are a reaction to the products transfused.
Collect
Whole Blood and Urine
AND ALL BLOOD PRODUCT BAGS AND TUBING FROM THE PAST 24 HOURS.

EDTA Pink Tube or lavender microtainer for pediatric draws, Sterile Urine Cup

(1 ml for pediatric patients)

Patient Preparation
A completed Transfusion Reaction Investigation, Nursing order must be sent ASAP to the Blood Bank OR a Blood Bank Transfusion Reaction Investigation Form # 7250.5d must be completed and returned to the Blood Bank.
